Pre-Matriculation Health Requirements

Incoming Goldman School of Dental Medicine students (predoctoral and postdoctoral) must obtain a physical examination and submit Boston University health forms PRIOR to orientation. 

You should make an appointment with your health care provider as soon as you receive your acceptance notification.

All students must submit a completed Health History and Physical Examination Form (Clinical). You must mail the completed form to Boston University Student Health Services for arrival at least one month prior to your program’s mandatory orientation. Indicate the dental school on the form by checking “GSDM”.

Deadline to submit the Health History and Physical Examination Form (Clinical) to Student Health Services:

Predoctoral programs: June 1

DMD and advanced standing: June 30

September start postdoctoral programs: August 1

 

You and your health care provider have different sections of the form to complete. Your health care provider must record your immunization history as part of this Health History form. Refer to specific inoculation information in the Student Health Services welcome and Pre-matriculation Information and Health Requirements document.

You may need to begin a series of inoculations now in order to comply with University and Massachusetts regulations, so do not delay.

  • Your doctor must provide your health and inoculation information on the form.

  • You need to complete certain sections of the form.

  • Make a copy of your completed report for your records before mailing it.

  • Send the completed form to Boston University Student Health Services.

  • Bring a copy to campus to ensure your health record is available.

  • Print the Information and Health Requirements for your doctor.
