Claim forms are necessary for members who choose to receive care at an outside center other than a BU Dental Health Center. The dentist must fill out the claim form. Click below to download the appropriate form (pdf):

• Claim form for BU employees only
• Claim form for BMC employees only

Note: no claim forms are necessary for employees of the Photonics Center and Skin Pathology Laboratory, who must receive care at a BU Dental Health Center.
